Hayward Evaluation and Response Teams (HEART), Case Management and Mental Health Linkages (LINK) TEAM provides homeless outreach, information and resources, follow-up support, and assistance with navigating resources We can also connect to family counseling, crisis intervention, and more through the Youth and Family Services Bureau (YFSB). Visit HEART on 1st and 3rd Tuesdays from 11am-2pm. What is the Community Hub? The Community Hub is a full-service, one-stop resource center for patrons to access City of Hayward and community partner services. Where is the Community Hub? The Community Hub is located at the Downtown Hayward Library, on the 2 nd floor, by the Tech Lab. Ask Library staff for directions! Who can use these services? Anyone!
